    Duloxetine for neuropathy

Duloxetine hydrochloride is a reuptake inhibitor of 5-hydroxytryptamine and norepinephrine used to treat depression, generalized anxiety disorder, neuropathic pain, and stress incontinence in women. We investigated the efficacy of duloxetine in painful diabetic neuropathy and fibromyalgia to allow comparison with other antidepressants. We identified six trials with 1,696 patients: 1,510 were treated with duloxetine and 706 with placebo. All patients had established baseline pain of at least moderate severity. Three trials enrolled patients with painful diabetic neuropathy (PDN) and three enrolled patients with fibromyalgia. The number needed to treat (NNT) for at least 50% pain relief at 12 to 13 weeks with duloxetine 60 mg versus placebo (1,211 patients in the total comparison) was 5.8 (95% CI 4.5 to 8.4), and for duloxetine 120 mg (1,410 patients) was 5.7 (4.5 to 5.7). There was no difference in NNTs between PDN and fibromyalgia. With all doses of duloxetine combined (20/60/120 mg) there were fewer withdrawals for lack of efficacy than with placebo (number needed to treat to prevent one withdrawal 20 (13 to 42)), but more withdrawals due to adverse events (number needed to harm (NNH) 15 (11 to 25)). Neuropathic pain is a chronic nerve pain that can significantly impair a patient’s functioning and quality of life. Neuropathic pain results from damaged nerves that have been affected by tissue injury due to trauma or disease. Diabetes mellitus and herpes zoster are commonly associated with the development of neuropathies. Other disease states that may result in neuropathic pain include complex regional pain syndrome, radiculopathy, and phantom limb pain. Neuropathic pain can also be medicationrelated, as in the case of nerve damage due to cancer treatment. Neuropathic pain originates from the reduced inability of a neuron to relay messages to and from the brain because of damage to the nerve. This can cause a person to feel a variety of sensations or hypersensitivities.

PURPOSE: This randomized phase III trial is studying duloxetine to see how well it works compared with a placebo in treating peripheral neuropathy caused by chemotherapy in patients with cancer. RATIONALE: Duloxetine may lessen peripheral neuropathy caused by chemotherapy. OUTLINE: This is a randomized, double-blind, placebo-controlled, crossover study. Patients are stratified according to prior neurotoxic agent (paclitaxel vs oxaliplatin vs other taxane agents without paclitaxel vs platinum agents [cisplatin] without oxaliplatin) and high risk for developing painful chemotherapy-induced peripheral neuropathy (no vs yes). Patients complete pain and quality of life questionnaires, including the BPI-SF once weekly and FACT/GOG-NTX and EORTC QLQ-C30 questionnaires, in weeks 1, 6, 8, and 13. After completion of study treatment, patients are followed for 2 weeks. Do not take this medicine together with bosentan (Tracleer®). Also, make sure your doctor knows about all other medicines you are using for diabetes, including insulin. Under certain conditions, too much metformin can cause lactic acidosis. The symptoms of lactic acidosis are severe and quick to appear. They usually occur when other health problems not related to the medicine are present and very severe, such as a heart attack or kidney failure. The symptoms of lactic acidosis include abdominal or stomach discomfort, decreased appetite, diarrhea, fast, shallow breathing, a general feeling of discomfort, muscle pain or cramping, and unusual sleepiness, tiredness, or weakness. If you have any symptoms of lactic acidosis, get emergency medical help right away. Hi I have been taking antabuse for a month and recently stopped. D I believe the half-life of antabuse (how long it sticks around in your system for) can be anywhere up to 13 days. I'm just curious as to whether your Dr spoke to you in-depth regarding the serious risks involved in taking it and drinking before you commenced the med? It's been awhile, but I may have even taken it the same day that I drank. Two and a half days after taking the last tablet i experimented and had a beer to see what would happen. In any case, I had no other symptoms besides turning red and getting a rash. I had not taken any pills for 7 days or so, an started drinking quite heavily. Disulfiram is the ultimate deterrant for me, never again will I play with fire. My face shoulders, chest and back had erupted in a a nasty rash. You can drink, or take antabuse - it's your call, but it's really dangerous to do both. Hi Chuck prescribed medications usually come with a little leaflet. I also believe the instructions say you should not drink alcohol for one week after stopping the medicine, as the reaction may still occur in this time. I have done searches and can't find anywhere that refers to this as a symptom. Didn't take long for my husband to figure out that I was still drinking while taking the drug, despite the original belief that I would be sick as a dog. I love the security this daily pill affords me, I am happy and sane now. I know, pot/kettle, but it's best not to experiment with this stuff.
